Cancer and Critical Illness Coverage for NVFC Members
An Optional Member Benefit
The NVFC has partnered with several insurance carriers to provide options to our individual and department members regarding cancer, accident and critical illness coverage. Please contact the carrier directly for information related to cost and plan coverage. Several plans also include an option to add a spouse and family members.

PROVIDER ABOUT THE PLAN
AFLAC Cancer Protection Plan Who is covered:
Individual or Department; No Restrictions; may add spouse to policy; Children always covered for free.

Benefit:

  • Plan pays cash directly to you in addition to any other coverage you may have.
  • One Price; Guaranteed lifetime rates.
  • $5,000 initial diagnosis per person; payouts increase annually for internal cancer.
  • $3,000 initial treatment for chemotherapy plus ongoing unlimited weekly chemo payments
  • $500 per week Radiation therapy and immunotherapy; unlimited benefit.
  • $10,000 bone marrow transplant payment
  • Up to $5,000 per surgery
  • $300 per day hospital stay
  • Hospice and nursing care benefit available

Other Items to Note:

  • Enroll before age 64
  • Lifetime coverage and your benefits are not reduced
  • Benefits are paid out over a lifetime of treatments
  • Policy has no deductibles, copayments, or network restrictions
  • $50 p/p p/year with the completion of an Aflac cancer wellness/screening exam
  • 30 day waiting period. Cancer discovered during this time will not be covered for 2 years
  • Initial diagnosis is not payable for any previously diagnosed condition or condition diagnosed in first 30 days.
  • Previously diagnosed cancer per covered person must be treatment free for 5 years to be eligible for coverage

AFLAC Accident Plan Who is covered:
Individual or Department, No Restrictions; may add family members to policy.

Benefit:
Lump sum payouts for treatments of injuries on and off the job and while volunteering. Pays for initial treatments and continues to pay for ongoing care.

  • Plan pays cash directly to you in addition to any other coverage you may have.
  • One Price; Guaranteed lifetime rates.
  • Accidental Death benefits from $25,000 to $100,000 no matter where accident occurs.
  • Medical expense treatments up to $10,000 for fractures, stitches, surgeries, dislocations, burns plus much more.
  • Lump sum payments begin with first doctor/ER visit and continue through follow-ups, hospital stays, ambulance, therapy plus much more.
  • Unlimited use.

    • Benefits include $1,000 for hospital stays plus per day stay, surgeries, physical therapy, and ambulance plus much more

ESIP Accident and Health Policy Who is covered:
Members of a fire department which includes the following: (the department would choose to cover the following).

Volunteers, registered junior members, any director, commissioner, trustee, board member, officer of the board or holder of a similar position; part time employees of the policyholder who work less than 1300 hours; bystander deputized by an official of the policyholder at the scene of an emergency, but only until the emergency ends.

Career members – a paid employee of the policyholder who works more than 1300 hours for the policyholder and is acting within the scope of his or her employment.

Auxiliary members – any person while assisting the policyholder if requested to do so by an officially designated member in good standing of the policyholder.

Benefits:

  • Death benefits available $10,000 to $150,000 (higher limits may be available) – death benefits are paid from death due to injury or illness as a result of a members participation in a Covered Activity under the policy.
  • Medical Expense benefits $10,000 – $150,000
  • Disability Income (weekly) $100/week up to $1200/week

    • Policyholder may choose benefits to be Primary or Excess over WC.

If a member has a Critical cardiac event, and elects this service, advocacy services can range from general overview of cardiac or burn care to heart transplant and burn units services getting your members to the best possible hospital for care, including Heart Transplant specialty hospitals. This service is available for events occurring ON or OFF duty.

ESIP Critical Illness Policy Who is Covered:
Active members of the emergency service organization. Coverage can be purchased by class and include spouse and/or children.

Benefits:
Up to $25,000 Accidental death and dismemberment benefit may be requested

  • No age limit but benefits reduce beginning at age 70
  • Benefit paid for Death or Dismemberment injury resulting from an Accident
  • Group Critical Illness Rider for Specified Diseases- Coverage Amount – 100% of Maximum Benefit Amount – based on limit chosen by Department – Benefit reduces at age 65 and 70> In-Situ Cancer> Invasive Cancer> Heart Attack

    > Kidney Failure

    > Major Organ Transplant

    > Stroke

    > Coma

    > Coronary Artery Bypass

  • Waiting Period – can be 30 – 180 days
  • Pre-existing Condition – 6 months prior to Effective date
  • Lump Sum payout after waiting period. No further benefit after diagnosis for any one of the above

Provident Critical Illness Who is Covered:
All active, rostered members of an emergency service organization in states where coverage is available.

Benefit:
Critical Illness Benefit options of $10,000, $20,000, and $30,000 including a $10,000 24-Hour AD&D benefit with each plan

  • No medical exams, physicals, or “knock-out” medical questions
  • Priced per person with no rate increase as you age
  • Benefit payments can be used for anything: medical bills, house payment, car payment, taxes, etc.
  • 90-day waiting period
  • Survivability – Insured must survive for at least 30 days after the date the CI is diagnosed.
  • No age restriction; coverage ends at age 75
  • Critical Illness Benefits for: Heart Attacks, Kidney Failure, Strokes (does not include TIA’s or Vertibro-Basilar Insufficiency) Invasive Cancer

All classes of members within the emergency service organization, to be determined by the organization. Including, but not limited to: volunteer, part-time, full-time, and auxiliary members.
